and that we hold on file a minimum of two separate contact details

e.g.,  parent / another family member / close family friend should we need to get in touch urgently

Please notify the school office immediately of any changes to home address, telephone number, mobile/work contact numbers of any person named on your child's personal details sheet 
by either sending written confirmation
